Before filling out a registration form, check you are in our catchment area.

Registering with the surgery

To register with the surgery:

• use our online form
• download and print a registration form [EXTERNAL PDF LINK], and bring it to reception during our opening hours
• visit reception during our opening hours and collect a paper registration form

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

You will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.

Medical treatment is available from the date of registration.  Please contact reception for further information.

New patients with regular medication are advised to make a telephone appointment with the practice pharmacist, more complex medication may require a GP appointment.
